Confusing time limit

It is very confusing how to fill out the FORM 8840. Say I arrive in the U.S. on November 15 of a given year and stay until April 28 of the following year. My question is, do I count only from November 15 to December 31 on one Form 8840 and then “save” the rest (January 1 to April 28) for the next year
